Does the Clinic accept health insurance?
YES. The Clinic is considered in-network with the following health insurance companies in the US.

Patients who wish to make a visit are encouraged to call and speak to the Clinic Manager. A benefit check will be done to determine the cost of a visit at our office for your particular health insurance policy.
Insurance card required
All patients are required to provide a picture ID (e.g. driver's license) and medical insurance card. At the initial visit, copies of these cards will be added to the patient's secure electronic chart. This is required to prevent insurance fraud and identity theft. At the initial visit, the patient must sign an agreement for assignment of benefits (payment) from the insurance company and release of information to the insurance company (in case of payment denial).
The Clinic accepts cash, Visa, MasterCard, Discover, and American Express cards. The Clinic does not routinely accept personal checks. If the patient sends a check and the bank declines it, the patient will be responsible for a $100 service fee to cover bank and reprocessing fees.
The Clinic offers laboratory services through LabCorp as a convenience to the patient. Patients are not required to use the Clinic's lab service if it is not convenient for them. All costs for laboratory tests are separate from any costs for an office visit.
